Shapes



Shapes, established in 2004, is our domiciliary service for children and young people offering flexible support for children and young people aged 2-18 years with individually tailored programmes. The children typically have physical and/or learning difficulties, many of which can be profound or complex.

Activities

Supporting Children and Young People

In the community e.g accessing youth clubs or leisure activities, enjoying meals out, shopping or going for a walk

In their own home at any time of the day or night

To develop as independent and social beings

Supporting Parents and Carers

With the challenge of continuing to care for their child at home

Giving them some free time to get out more easily – if only to do a food shop, have a break during the day with friends or spend time with other children

   

Professional support

Each family who accesses Shapes has a small team of fully trained regular staff, as determined by the needs of the child.  These needs are regularly reviewed, re-assessed and adjusted as necessary.  This relationship enables staff to know the child/young person and their family well, and allows a trusting relationship to be built. Staff provide support in all areas of personal care and this could include medical areas such as:

  •  gastrostomy care
  •  complex epilepsy
  •  oxygen therapy
  •  oral suction

Shapes works closely with Smithers, also part of Diverse Abilities Plus, a short stay residential home for children.

Community Links

Close liaison is maintained with social services, schools, community nurses and other professionals to ensure that Shapes, along with Smithers, is meeting the needs of each child.

Shapes is regulated by the Care Quality Commission and follows Domiciliary Care Standards.
